SERVICES OFFERED

Care Plan Oversight

We oversee patients who are under the care of home health agencies or hospices, developing or revising care plans, reviewing patient status reports, analyzing lab results, communicating with other healthcare professionals, integrating new information into the plan, and adjusting medical therapies as needed.

Transitional Care Management

Our team provides communication and visits within 7-14 days after a hospital discharge. This service is designed to minimize the risk of re-hospitalization during a vulnerable time, ensuring continuity of care after you leave the hospital.

Chronic Care Management

For patients managing chronic conditions, we offer enhanced oversight, including regular phone calls to check in on your status, frequent adjustments to care plans, and other services to help maintain stability in your health.

While MK Physician Associates does not provide direct laboratory or x-ray services, we collaborate with local providers to ensure these services can be performed at the patient’s residence when needed.
